Fireup Soccer, LLC - Player Waiver 2025

By participating in any activities or services provided by Fireup Soccer, LLC, the participant (or their legal guardian) acknowledges and assumes any and all risks associated with the activities involved. This includes, but is not limited to, the risk of personal injury, property damage, or any other loss that may arise during or as a result of participation.

Liability Waiver and Release Agreement: The participant agrees to release, indemnify, and hold harmless Fireup Soccer, LLC, its owners, employees, agents, and affiliates from any claims, demands, or liabilities, including legal fees, arising out of or in connection with the participant's involvement in Fireup Soccer, LLC's activities or services. Furthermore, they acknowledge and agree that their information may be housed and utilized by the organization for administrative, communication, and promotional purposes related to the activities and services provided.

Media Release Consent: The participant (or their legal guardian) grants Fireup Soccer, LLC permission to use photographs, videos, or any other media taken during the activities for promotional and marketing purposes, including but not limited to website, social media, and printed materials. The participant (or their legal guardian) acknowledges that Fireup Soccer, LLC is not responsible for any injuries, accidents, or damages that may occur during training sessions, events, or any related activities. It is the participant's responsibility to maintain appropriate health, fitness, and skill levels for participating in the activities. Furthermore, the participant (or their legal guardian) agrees to comply with all rules, guidelines, and instructions provided by Fireup Soccer, LLC during the activities. Failure to comply may result in the participant being removed from the activities without any entitlement to a refund. This liability clause shall be binding upon the participant, their heirs, representatives, successors, and assigns. It represents the entire agreement between the participant and Fireup Soccer, LLC, superseding any prior discussions or agreements, whether oral or written.

By checking the registration form box, the participant (or their legal guardian) acknowledges that they have read, understood, and voluntarily agree to the terms and conditions stated in this liability waiver, including the media release consent.

By checking the registration box, the participant (or their legal guardian) is delivering an electronic signature that has the same effect as an original manual paper signature. The electronic signature is equally as binding as an original manual signature.
